Tang problems

I have had several problems keeping naso, kole eye and blue hippo tangs alive. They do great in qt but once in the display tank they die with a week if I’m lucky. I only have 3 clown fish in my main tank. Please help! I have a reefer 450xl, set up in 8/2020....

If it happens frequently it might be worth trying a different supplier, assuming your water parameters are all ok

This fish appears to have been overcome with velvet and common with surgeonfish - generally velvet or ich

Welcome. The fish when they go into the DT may be ok after appropriate QT (i.e. TTM, etc.) But the DT likely has disease. Fish looks like Velvet especially in view of rapid onset and death. 72 day fallow would be prudent.
