Does this tank need to have a flat surface under it?

I don't think so. It's made to sit on the plastic trim. You should only need support around the four sides.

This had a 125 gallon on it for years, and I added a back and side brace. What I was reading is the rimmed tanks don’t need the same support and this should be fine. Thank you!

My concern would be that while it's bottom does not need to be supported, the rim itself does. If the rim is hanging over that hole, then it is not supported along most of its length. Maybe it holds, but I wouldn't bet money on it.
