lower half of tank for sure. enough flow to prevent detritus build up, but too much flow will prevent polyp from fully extending.

Depends on what you want to stock aside torches. I placed mine on top of the rock work as I have no plans on SPS. I just tuned the light intensity to suit the torch placement in my tank.

here my tank :

@Niam has a good point- The torch tentacle can have quite a reach and potentially sting others. So you may want to find an area large enough for it.

Due to space constrained I cant have a bigger size tank. I do enjoy the current 45 gal tank though. BTW may I ask, what are you battling with when your nitrate is high, (algae? coral coloration? coral dying?).

I mean when your nitrate is high how does it effect your tank? is it algae bloom? or Coral dying? or fish dying? the reason i asked is because, NO3 at 4ppm is not a big concern for me as it is controllable.

Ouch.. that hurts. I have 0 experience on SPS, I cant comment much but in my LFS SPS tank, the nitrate is around 10ppm. Here is the photos i took from my LFS SPS tank (sorry for bad quality photo taken without lenses, the lights are too blue). I think beside NO3, there must be some other element that causes your SPS to bleach and die (i'm not too sure though).
