Oh yikes! Poor little guy! Tube anemones really do better when the tube bit is buried in sand. Could it have cut itself on the rock? Or perhaps been attacked by a crab or shrimp?

Yes he should be buried and if u can't bury him then use an adequate sized piece of PVC to place him him...which leads me too why is the PVC on his head????
