Twin Spot goby starving

Ok so bought this twin spot goby. My tank has been up and running 1.5 years or so. Thought I had enough food for him in the sand. He is activley sifting but seems to be loosing weight. What can I do to help him put on some weight?

What sized tank?

May also have internal parasites, treating for that may help.
