Unknown Growth / Pest / Disease on Torch?

My torch seems to have weird squiggles near its mouth, similar to what I’ve seen on POed mushrooms... anyone see this before? It has been extending very well recently, so I’m not sure if this is stomach contents because it’s mad, or if it’s waste? Some pieces seem separate from the bunch. I fed heavily last week. Before anyone asks, I’m not sure what my params are - I just perform weekly 20% water changes, skim heavy, and use carbon. Every other coral is fine....

Decided to test:

SG: 1.025
Temp: 78
Nitrate: 5 ppm
Phos: 0.01
Alk: ~9

It’s much more extended now, with exception to the few tentacles near the mouth. Flow is uplifting and bounces off the glass with a spinstream in an IM NUVO 10.

Am I just overreacting?
