Uronema? Pics attached

This fish looked completely healthy, was active and eating 6 hours prior to this Pic. It has been in a QT tank about 5 days, since I got it and has not been near my display. Im pretty sure this is internal uronema and this fish is beyond help. Should I euthanize?

There's also 5 more brand new living anthias with no symptoms in the same QT. QT is cycled and theres 0 ammonia.

Uronema is oval and ciliated but this may be the start of it. How were they acclimated and did you quarantine them?
With these fish, I highly recommend a 60 minute ruby rally pro bath and qt with formalin base

I only temperature acclimated them. I did not drip acclimate them but, like I said, they've been fine for several days.

They have been in the QT tank for 5 days. I have only done 2 rounds of metroplex and kanaplex so far. I was hoping to not have to go the way of a formalin QT, although, I have prepared for it and am ready for it just in case.

Im actually setting up the formalin QT in my garage now. Is ruby reef ralley bath a must or could a 175-200 ppm formalin bath followed by 25-30 ppm formalin QT for 10 days suffice? I only ask cause I dont have any reef rally

Poor acclimation was likely a contributor and others too should have suffered. You can not acclimate them properly and hope for the best. Best remedy for uronema is prevention. The key cause is often low salinity which is a step you missed as well as clean tank as uronema can thrive on bacteria, uneaten food, and waste so besides treatment also maintain a clean tank removing waste daily.

Internal Uronema more typically follows a fish’s muscle pattern, a thinner, linear lesion angled towards the head.
However, the time frame for this lesion is consistent with internal Uronema.
