Who's the primary suspect?
Primary suspect is a blue leg hermit I've seen hanging out on there before. I never saw the hole though with the polyps extended. I just noticed last night when I was doing a later water change.
Do you have a peppermint shrimp?
Nope. Blood cleaner shrimp and a scarlet skunk cleaner, and a tiny snapping shrimp.

My guess is it's an infection of some kind or dieoff from tissue being pinched internally as it's growing and not someone chewing on it. If you gently siphon out the hole you'll likely get a lot of loose tissue and gunk.
