Water change

I set up my salt water tank five weeks ago. The cycling took two weeks and then I added two clown fish and 1 fairy wrasse. I have not changed the water for the last five weeks because my nitrates are at 5 ppm and my phosphates are zero should I change the water since it been already five weeks with the same water.

I found my wrasse hiding today and didn’t even wanted to come out to eat. I’m worry the water might be an issue??
 
I have a small Duncan and two tiny zoas as well.
 
Fish only need ammonia in check, pH and Salinity. Other numbers don’t matter. Wrasse is stressed out that is all.
