What does this look like to you?

I’ve been struggling keeping acros and some sps species for awhile now. I’ve included a few images to explain the trouble.
system is 2 years old, run T5/led combo ( par readings are lower 250ish at most),
Parameters:
Gravity 1.025
Nitrates 25ish
Phosphate.15 ( both nitrate and phosphate are higher due to Cyano issues. Tank is clean but levels remain higher. Some sps are fine)
Alk 9.5
Calcium 450ish
Magnesium 1350ish
Does recession appear to be parameter related or maybe pest issue? Curious what people think?

Stability may be the issue. Is your alk stable?
Also, you may want to send in an ICP test to see if there is something in there that you're not aware of.
PO4 and Nitrate are high. Want to shoot for about .05 PO4 and 4-5 Nitrate.
 
Alk is pretty stable. Nutrients were higher to rid tank of Cyano. ICP is a good thought. Will get one out next week!
