The last picture is of it yesterday before it started this abnormal behavior. Is it in pain/is there anything I can do to help it?

it isn’t dead as it’s tentacles move when I touch it but this doesn’t look like healthy behavior.

Tank age, lighting used, alk. Salinity, nitrate and phosphate? Preferably with a refractometer that's calibrated and tests that aren't API.

It appears to not be able to find a spot it likes and is tumbling in the current. When settling in I tone down my lights or they will run for a cave and not be able to receive light alot of times. If you can lower your light setting and try and place it right where the sand meets some of your rock. They LOVE to anchor right where rocks meet sand.

its not a good pic, hard to say whats wrong with it.

When did you get it? Did you put it on a rock and hold it down for minute? Usually it will attach to the rock when you hold it. Has it moved since you put it in the tank?

Really need more info

I am not sure from the pictures, but best thing I have found is turn off flow, put it on a rock or between a couple rocks gently (can hold down by mouth for 30 seconds or so on rock), leave for a bit. Come back and check. If it is attached it will open a bit or tuck in a bit and you can turn flow back on.

the tank is just about two months old.
Nitrate: 10-15 ppm (a bit high because I just re-arranged shells)
Nitrite: 0 ppm
Ammonia: 0 ppm
PH: 8.1
Salinity: 1.024
Phosphate: 0.25-0.30 ppm
Calcium: 480 ppm
it looks even deader now but it still reacts when I touch its tentacles :(

I got it from saltwaterfish.com and it was one of only two organisms that came alive. I put in on a rock and it stayed there for a bit but then started meandering about. When it stopped meandering it just started curling in on itself and kind of turning inside out.

2 months is a little early for nems. Also didn't list alkalinity
