My initial guess is anemones mushroom/corallimorph

It is a Corallimorph, not doing so well though. I'm guessing a very bleached Ricordea yuma.

What can I do to try and help it

Just give it time in a stable system and food if you can. Did you just get this mushroom? If so, just leave it be, don't blast it with too much light and it should recover. They're pretty resilient
