White spots on gems tang

Noticed a couple spots on my gem I have two wrasses
Could they be picking off scales ?

That looks like Lateral line disease, supposedly linked to charcoal filtration.

Are there similar "spots" on both sides of the fish?

Do you mean the chain of white spots? Those are a stress reaction that this species can show. You may see them at night as well.

Agree on stress lines and mine shows this as soon as lights go on each day
