Why does my Duncan look like a plate?

My Duncans has retracted all of its tentacles, and just looks flat now...
Its been like this for 1.5 months now... but yet its still growing new smaller heads..
Alkalinity = 9.8
Calcium = 480
Magnesium = 1500
Nitrate = 11
Phosphates = under 0.25
Ph = 8.1

Looks like you have dinos in your tank. How old is it? I'd start there. The dino's are likely starving it.

the tank is 4 months old

Very likely you have an underlying nutrient issue which is causing the issues with you corals. Dinos are common in new tanks because they lack nutrients. Id start by vacuuming out as much as you can and checking out the dinos thread.
