wrong hole ! apex el help

I noticed my mp 40 went off and no light on the eb8 so I tried turning the output off and on from the fusion app .still nothing . I noticed the output name changed to next output (which was off ) . I turned the output on and now it works it still reads the wrong outlet .What happened ?how do I fix it ?
