Yellow tang HITH or HLLE?

I purchased a yellow tang on September 3rd. Overall seems to be doing well but I noticed a hole right in the middle of the head, I'm wondering if this is hole in the head disease or lateral line erosion. Ammonia and nitrates are not elevated. Salinity is stable. The fish is eating and swimming around normally. All other tank mates appear fine.
The only major change recently was critically low alkalinity so I added two tsp of baking soda to the tank. It's a 90 gallon with a 20 gallon sump.
Should I pull the Tang and place in a isolation tank and treat with paraclense which states can we use for salt water.
I just did 1/3 water change. Theres nori sealed available.
Need to know what to do.

Sorry to hear about your issue. I would suggest posting some clear pics for the fish medic team to take a look 🙂

This fish Should be quarantined anyways, so isolation, yes.
Pics under bright white light intensity will help determine what you have, no blue light please. Skip paracleanse which does nothing for most fish. Best is coppersafe at 2.25ppm for 30 days and even follow with two applications of PraziPro thereafter.
Is fish breathing normal or elevated>

What foods are you offering it?
Its appears completely normal outside of the hole which is concerningly deep in appearance. Its difficult to get pics.
Outside of natural grazing, it has nori and micro wafers for tropical

This looks to be a significant injury or even tail strike from another tang. Quarantine and observe and be prepared to treat it should it become red or infected with Seachem Kanaplex with air stone added

This is not HLLE / hoke in the head. Looks like an injury of some sort.
