Yellow wrasse issue

So my Yellow Wrasse has just came out of the sand bed one week since introduction in to the DT. The wrasse is not laying on its side breathing. Any ideas on what is going on? The wrasse was very lively during acclimation.

I'm not expert but FWIW: Could have been uronema based upon the spot and how rapidly it died. :(
