I'm indeed disappointed and surprised lol! What happens, is there CO2 getting the water? Or something else at play?

They sometimes just aren’t dissolving calcium hydroxide fast enough to keep up with water flowing through it.

EDIT: I wonder if this would indicate a smaller than necessary stirrer body for the amount of effluent being dosed. Perhaps a larger stirrer would solve this.
 
Ive not tried to figure out how to make one, but the difficult trade off is getting fast enough mixing to saturate while not getting solids into the outlet.

I believe you run them on a timer so that they mix right after it's dosed, to give it time to mix and dissolve, and then settle out. Are you maybe thinking specifically of the Two Little Fishies Phosban Reactor that doesn't stir, but uses the inward flowing water to agitate the undissolved Kalk? In that case, I can see the "storm" of Kalk in the reactor being added to the tank if a large enough volume of Kalkwasser was being dosed.
