Hello!

So looking for some advice.

It seems like for months and months now we have been dealing with some kind of bacterial issues in our tank. As far as as we can tell it’s not affecting all fish, just some new ones that are added. The issue seems to be a mixture of stuff, from looking at the fish it looks like fin rot, pop eye, and some kind of skin infection, symptoms same across all fish that get this issue. We have lost several new fish despite over months despite treating tank with advice from LFS.

Tank info: 3/4 months old but was an upgrade so cycled with old tank media and water.

Stock: currently - yellow face goby, pink pinstripe wrasse, royal gramma, flymo blenny.

Royal gramma and flymo blenny infected - lost blenny today :(

Tank parameters:
Salinity: 1.025
Calcium: 395
magnesium: 1395
Phosphate: 0.65 (bringin down with RowaPhos)
Alkalinity: 7.2

We have and are dosing: oodinex (reef safe) and polyp lab medic

Have a few pics but will add more as I can get them.

Thanking you!

This is likely scratching from flukes for which these fish are susceptible to. The meds you are using are both considered useless and Medic is Peroxide salt at a very high cost of $40 per bottle. Most any med that is sold as safe for reefs is safe for protozoans. They simply dont work.
Other signs of flukes is yawning, sudden darting, itching, elevated breathing, loss of appetite and loss of color as examples.
If flukes, you want PraziPro which is safe for most specimens but also effective and dosed at 85% of recommended dosage with air stone added during treatment. If no PraziPro in your country, you can use Sera Tremazole at full dosage

Welcome to Reef2Reef!

Pending some clearer photos (video would help also) this sounds more like flukes than a generalized bacterial infection. Most bacterial infection are started by some injury to the fish’s skin.

Here is a post I wrote up about using praziquantel to treat flukes.
