Black leopard wrasse white spots

My wrasse came out today and its mouth looks white with a white spot around it. There is also some elongated white spot on its fin and its back fin seems like it’s being eaten away. He was perfectly fine yesterday and did till eat today. I ordered prazipro thinking maybe it’s flukes?

Can you please take some white light photos and a short video if possible please?
 
Pics are hard to make out, but it sounds like a bacterial infection from an injury. Treatment with kanaplex in qt should help.
 
Pics way too dark but appears to be damage and can be from rock or running into object
Is fish breathing normal and attempting to eat?
 
If mouth damage as suspected, treatment in a separate tank will be seachem neoplex
