I got this new blue tang a month ago, it's kind of healthy for the first two weeks, then got ich, then turned like this.
Treated with H2O2 dip, neomycin, metronidaole for 5 days, it got worse😂, I wonder if these are scratches from the hide being too tight? But these look like some overgrown and can't scratch off.
Can you help me identity this disease?
Grateful for any idea or suggestion!!

This is a result of stress or skin irritation which these tangs occasionally get and can be mixed in with ich
They respond well to coppersafe at 2.25ppm but you can wait and see if it goes away
If not, treat with coppersafe and monitor with Hanna brand test kit
You will notice other fish show no signs but consider treating them also
Looking at tail, there looks to be a bacterial injury and treatable with seachem kanaplex

Can you give the rationale of what was going on that suggested H2O2 drips, Metronidazole, and Neomycin - or were the marks that are on the fish there already. Only asking because many times chemicals (especially multiple chemicals) can actually irritate the skin by removing some of the natural bacteria on the skin.

But if I'm understanding what you're saying - the first thing was ich - which is normally treated with copper. Did you quarantine the fish at all? How big is it? What happened exactly after the ich? It looks in the one picture like the lesions are flat on the body - and then in the second almost 3 dimensional. Can you perhaps get a better picture - or describe?

If the lesions are on one side only injury is suggested - whether those have become infected secondarily is hard to day. I think there is also secondary mucus formation due to the chemicals

Many thanks to your help!!
My other fishes show no sign of ich or skin lesion. The very first lesion came when this fish got ich and felt too itchy to scratch on rocks. Most injuries are on the same side, including those tail injuries, i gave it a small hideout with a small entrance, I presume might be the reason of those injuries.
I'm planning to listen to your advise and change the antibiotic to kanamycin(same amount as kanaplex), and give it a relatively large qt tank and hideout. If things got worse, then use coppersafe treatment with cautious.
Thanks again!!

Hi! Many thanks to your concerns!!
The H2O2, Metronidazole, and Neomycin are for antibacterial purpose, H2O2 is 5ml*3% per liter saltwater for 30min, and Metronidazole, and Neomycin are the same amount suggested by seachem metroplex and neoplex.
The fish is 6-7cm long. Many people suggest that a few ich on blue tangs can be self-limiting, so i didn't take measure to treat ich, and the white spots did get fewer. But the fish did get itchy and scratch itself on rocks, that cause the first lesion, I presume. The lesions mostly are on the same side i showed in these picture, its 3 dimensional ,likely to be mucus. I highly suspect that it might be the small hideout with a small entrance that cause these scratch injuries, the blue tang is very nervous and keep on scratching, adding the secondary mucus formation you mentioned. I'm planning to change the antibiotic to kanamycin(same amount as kanaplex), and give it a relatively large qt tank and hideout. If ich comes back then use copper treatment.
Thanks again!!

One side suggests as suspected stress marks and skin irritation as suspected.

The three dimentional aspect is odd - unless it's just mucus. If it's actual tissue that was scraped off the body, thats another story. Agree with your plan for an antibiotic - though this may be one of those rare instances that there is a fungal component. Would watch carefully - and good luck
