Carpet help. It's STILL sick

I've had this nem for close to a year now and it was doing amazing for a long time. Then I got a new carpet in which ended up getting a bacterial infection. It passed it to my third carpet and malu, killed them both. This big green one was a trooper though,made it for a long time looking decent but eventually started to look like it had an infection. So I treated it with cipro, and by the end of the treatment it was looking awesome again. Buy back in the DC it started shrinking and deflating again. It's been doing this for probably 3 months now. It was my understanding that infected nems will not last that long. I'd like some opinions on what could be going on. It eats very rarely, still has decent feeding response although most time it doesn't consume the food. I try mysis and krill.

How it looks on a daily basis. It's in my frag tank now.

Have you tried to place a lager enough container with enough fine substrate in there for it to bury its column in?

I've had it near the sand in my display tank. Enough so that it's body was touching the sand, and it never moved from its cup.
