Clownfish not doing good

Any ideas what I’m dealing with here? couple hours ago noticed one of my clowns clowning around and hanging out near the top more than normal. Took a look at him and he had some white blotches. Watched him for a bit and saw him go down to the sand and lay there so maybe it was just sand stuck on him. Then I look at him again near the powerhead and all of a sudden he starts getting blown around the tank uncontrollably end to end (150 gallon).

I just netted him and put him in an isolation box without reef lights to examine. I don’t see as much of the white spots here which makes me think it really was sand. But now I’m seeing other colors on the rear left side and he’s just bouncing vertically and isn’t swimming normal at all. Looks like it’s taking a lot of effort just to stay vertical

Any ideas what I’m dealing with? All my other fish are currently actively and looking fine

Is this a new tank? Have you checked your ammonia level?

Been running for a couple months now. Water is good, ammonia and nitrite both 0

This does seem like a possible pathogen. There's an R2R Fish Medic team that may be able to help, but I think to better help them help you it would be best if you could take a short video of this fish using the brightest white lights you have, turning down all your blues for clarity. It's best if you could first post the video to YouTube, and then link it back here. This way, all R2R members will be able to view the video. Then, the #fishmedic team may be able to take a look and offer a diagnosis and treatment plan if needed
