Assuming stable living conditions, I guess I'd start thinking about possible pests.

I have coral rx is revive better ?
I have never had pest on my torches, it’s usually your tank parameters and bacteria infections. Revive fights off bacterial infections better IMO! Bu if you think this is pest related I would think rx is best, but to me, it seems like a bacteria thing. Torches are hyper sensitive.

Used revive still not opening fully. Did notice it looks like it’s splitting into 2 more heads do they act differently while splitting?

Most certainly they act differently when splitting. Typically they stay shrunk up when splitting. They will still inflate, just not as much as usual.
Same as if a hammer was splitting! Just watch the torch carefully, have you seen him eat anything? I hand feed my torches via a syringe, any meaty food works.

Half of it looks fine other half where I think it’s splitting sucked in
