What do I need to do to save my coral. It has been like this for some time. It looked fine for months then this?

It makes sense to post a pic and some nutrient test results.
Elegance are tough corals to keep and like the water a bit dirty.
 
I am currently trying to figure this one out as well. I just received a really nice aussie Elegance last week. Acclimated it and everything as usual in my frag qt tank. Some reason it receded and melted away wuickly. Just pulled it out of my tank today. If I come across anything groundbreaking I will try and help
