Anything that would pick at it? Angels, tangs, crabs? Also check at night for predatory worms.

The tangs are picking at it now. It has had periods where it was closed up for a week then it would be ok but this time it's been two weeks and now it's falling apart. Nothing else in the tank is showing signs of stress.

not sure, I have several, and my largest one has been in a state of decline for almost a month now, I just moved it to another tank with indirect flow, looks like it has slowed on the decline, but still looks pretty rough. anyone else have the same problem? solution?

+1 Also check for leather eating nudis. When I find them they are mostly on the stalk of the leather eating into it. They blend in perfectly so taking a soft toothbrush and going over the entire thing gently is the best way. Just look for parts that look like they'll peel off or aren't firmly attached and that might be the issue. GL. Hopefully it's just needing to shed or something simple.

I've had this happen. I think this is caused when too much algae grows too fast on its tissue. Normally, the coral will close up, create a waxy film, and shed the algae with the wax. Sometimes it has trouble shedding everything and starts to suffocate. What I did to save it was easy, you just have to get wet 2x a day. Just stick your hand in there and massage the disk with your fingers. Make sure any of the dead tissue gets rubbed away. Do this 2x a day until it can get rid of its waxy coat and there isn't any more decay. Wouldn't hurt to put it into better water flow too.

I would try adding some new carbon for a little while too, as a distressed leather can cause water quality issues.
