Flame angel in trouble...

I went home for lunch and saw my flame angel breathing hard and fast while on its side. All the other fishes are fine. Only thing I did was my bi-weekly water change of 15 gallons, changed my 2 month old GFO via reactor and added more flow inside the tank. I also added an amp of Biodigest(my 6th week). Nothing out of the ordinary. Any ideas? Only parms I was able to check was my salinity(1.026) and temp is 78.5. I'll be checking the rest later tonight when I get home.
 
It could be lack of oxygen. Might try to put him box with holes (put inside your tank) and put air pump in it.
 
R.I.P. Flame Angel. It was still alive last night but did not make it this morning. I'm down to 3 fishes but on the bright side, more PE for my SPS. Any ideas and suggestions for a new fish?
