Tank looks clean. How old is the system?

The only thing he does is brush against the sand like at the end of the video. He does it pretty often, but here is another video

OH YEAH. That video is a whole different story. Why not just get some API General Cure and treat for Flukes right in the tank. Most definitely add a large air stone.

What does the air stone do? Never treated flukes before.

Beefs up oxygenation during the treatment process. Your local chain pet store should have API General Cure. Dissolve one packet in RODI water 'per gallon' (of tank volume), and pour into the tank. Day 8 (note: different from what the box says), do a 20% water change and dose again the same way.

My tank is 20 gallons… what should my treatment schedule look like? Is it reef safe?

Yes, I have a tang, and eel, and 2 small hermit crabs along with some anemones

The API General Cure is generally considered safe for all of that. If your tank is 20-gallons, factoring in rock, sand, etc., I'd probably dose a packet and a half (with the increased aeration!), then the 20% water change and the other packet and a half on Day 8.

Can it be soaked in food? That would be preferable over dosing the whole tank

I would have to say no. At least it would be beyond my personal advice & skill level. If it makes you feel any better, I have two Butterfly fish on Day 7 of General Cure right now, in a tank with rock and sand; usually I use an 'empty' quarantine tank, but not this time. Lol.

I’m being told it’s most likely ich my a Hawaiian fish guy, and I caught this tang from Hawaii tide pool… I will prob dose coppersafe and remove all inverts

That's even better, to be frank about it. But if you use that tank (with the sand and rock), you will have lots of problems with the Coppersafe being absorbed. And will have formidable issues housing inverts and corals in the future. Ideally you want to (1) Get "all" your fish out, and treat them separately in a QT, and (2) keep the main tank fallow to kill off what it's currently infected with.
