Hi everyone! Been a reefer for a while now, about 7 years. I've had this 90 gallon setup for 4 years now. Been going super slow on adding things to it. But my fox face has been acting strange for about 3 weeks now. Been bumping into everything, eyes are very hazy when the lights are on. Now he has sore spots above his eyes that you can see in the picture. Wont eat even if I put nori right in front of there mouth. Has no fin rot or white spots. But its face is starting to get pretty beat up with running into everything. I took some photos. I've done everything I know to do, like water changes, dosing paraguard. Checked my levels all of it. Now I did add a green star polyp and a montipora a week before this started happening. I hope someone can lead me in the right direction.

These spots which appear to be flesh erosion and may have stemmed internally from a worm such as nematodes and even coccidea , both internal parasites which can cause neurological damage or infection which likely has affected its sight and would have occurred over time and may have been preventable if addressed sooner. The erosion has also approached the eye sockets and i am less than doubtful if any treatment can reverse or even stabilize this. The eyes do have signs of slight exophthalmos, causing the eyes to slightly extend.

How long have you had the foxface?

He's currently got some little air bubbles on him.

Everyone else in the tank is looking healthy and eating great.

I don’t see any other issues with the fish - no eye damage flukes or rapid breathing. That kind of leaves some neurological issue. Those don’t have any treatment though, sorry.

Is that something that just happens sometimes for no apparent reason?

Yes, fish medicine isn’t very advanced and we lack diagnostic equipment, so there are many issue where the cause can’t be determined.

After another week, if no improvement and if you can’t get it to hand feed, I would consider euthanasia -
