Gaping lps mouth

One thing it looks to be doing is forming a new head; are you referring to its tissue or mouth?
 
It looks like a blasto to me. Recently purchased one and presented the same gaping mouth on night one. Blasto went back to normal next morning. This is only assuming it's new in the tank. If not, something might be stressing it.
