Help... Torch Coral Leaving?

Have had this gold torch for several months now and always seemed healthy. Just yesterday noticed that one head/skeleton is exposed and seems to be melting or leaving... any thoughts?

Hard to see anything wrong there.... wheres your nitrate and phosphate sitting? I tend to lose heads of euphylia when phosphate bottoms out.
