help with LPS

I received this LPS for free from a fellow reefer. It was covered by another coral in his tank. It was given to me as a hammer. I really don’t know what it is and it doesn’t look like any the hammers I’ve been looking at online and in the forums. It’s been in my tank for a little over a month and has sprouted two tiny new heads. I feel like there’s something wrong with it because some tentacles are more extended then others, some are darker in color and some are almost white. The tentacle sizes and lengths are also completely different.
