Hole in head??

So after watching my tank like I always do I looked at my clown fish and it looks like a tube or something is coming out of her head. My first thoughts were hole in the head disease but I'm no expert so I'm here to ask what is it. Had this fish for over a year (only fish in the tank) still eating just fine, though she does jerk around a little. Seems like something is bugging her. It's the little whitest spot above the yellow face and just below the white stripe.

No white stringy poop, normal brown still, but kinda looked stringy, guess it's just intestinal irritation? I did feed a new food yesterday for the first time possible cause maybe?

Very possible. Hole in the head is caused by an internal parasite (Hexamita) which also causes the fish to have white stringy poo.
