Ick , Velvet or ????

Good afternoon,

Not sure what I'm dealing with, My fox face (Morimer) tends to hide and only comes out to eat. I have no idea how long hes been like that. My wife notice the brown/black spots on the fish. Any ideas? The white spots at the tail end are food that i used to entice him to come out

Are the spots raised a bit? Looks like it on its face. Look for tiny worms with forked tails.

Thanks for your help! To me they look like sesame seeds. Last night they were all gone.

I try to catch him and I couldn't so i removed some rock and I guess I over worked him or gave him an heart attack.. he just went kaput. I ended up emptying my tank and I'm going to "reset" thank you for your help!
