So my toadstool leather has been shrivelled up and unhappy for a over a week or so and it’s just looking worse and worse. I’ve tried water changes and adjusting the flow. Is it too far gone at this point? Is it a disease?

It's not ded, you can even see the polyps coming out a little bit :) just give it time it should bounce back. Leathers do like some nutrients in their water though. I'd suggest testing your nitrates and phosphates
 
Do you have a list of parameters from the last time water was tested ?
Leathers do shed and wilt from time
To time .
Is it possible something in the tank is irritating it ?
 
HI!! Its not dead. They do shed the skin and tend to close up for a period of time. Try moving it up off the sand. Attach it to live rock or a frag rock. Urchin will definitely mess with any corals that are not glued down.
