Is my tank diseased?

Long story short. My tank is almost 1 year old. I lost 2 helfrichi and a royal gramma. I'll include pics of the dead fish from a little while back. My midas blenny would always flash, scratch, and bite itself. Also my gramma would flash and scratch its body on surfaces before dying.
After the gramma and 2 helfrichis at different times died the blenny slowly stopped flashing. For the past month all 5 remaining fish have been well behaved with no flashing or scratching. Blenny was even perching on rocks relaxing. All of a sudden I find my blenny marked with purple bruises. The next day I find it dead on the sand bed. There is no aggression from any of my fish ever. Do I put my remaining (royal gramma, melanarus wrasse, 2 clowns, yellow watchman) into a qt tank and dose with copper and let my display go empty with coral only for a few months? What could this be. The blenny dead was rhe last one to go. He does show discoloration in the Pic you see. I also included my last water test the day the blenny died. I know phosphates are high snd they will be lowered with phosphate lower thing. Its worked in past

The skin biting is a symptom of flukes.

Sounds like you need to treat all the fish in a QT with Coppersafe for ich and also praziquantel for flukes.

No the blenny only had that purple on it then next day skin discoloration before dying

Is there a reason my melanarus and clowns have been fine? Or is it just a matter of time before they die? They all seem pretty normal for the most part other than wrasse swimming against glass I think at its reflection. My fear is to go through all this process only to put my fish back in or new prequarantined fish back into my display and then same issue happens and all that work was for nothing....what are your thoughts

Different fish have different susceptibility to ich, but All exposed fish need to be treated even if they aren’t showing symptoms (yet).
