Long Tentacle Plate Coral- Having Problems

I am having serious problem with my long tentacle plate coral. Does it look as if it is dying. I tried feeding it. My phosphates may be a little high but other parameters are okay. Should I give up on it and take it out of the tank?

why give up on it, ts still alive. And plate coral is unique it can sprout some babies months after it has been dead, so leave it in the tank even if it dies.

If you can share more details about your tank (parameters, tank age, lighting...) I'm sure folks will be able to help.
