Montipora Capricornis changing morphology

Hey guys,

My Montipora Capricornis is my eldest coral (5 years) and has been truly abused over the years, but always grown in a typical plating fashion. After the last incident over a year ago, growth has been stunted until a few months ago.

All new growth has been of the branching type and has recently developed white growing tips with white flesh and polyps.

Radion XR15 Gen 4 AB+ program. Light has been on the same schedule for the last 4 months

Temp 26c
SG 1.027
Phosphate 0.05
Nitrate 5

Has anybody seen this before?

Mine did that one time when it got ripped apart from trying to catch some fish. Eventually the "fingers" grew together forming a weird looking plate.
