You’ll need to confirm if it has ich or not, posting a white photo/video will help. That disease can be fatal pretty quickly.

Yes, that confirms you have ich in that tank and that with fish loss, it has gone beyond the probability of just managing it, you’ll need to treat it. That’s going to be tough - any invertebrates present will rule out hyposalinity or copper treatments. Some people can move their inverts to another tank, other people need to move the fish to a treatment tank - each situation is different.Hope you can see better in these photos.
That diet will be inadequate for a moorish idol. They are omnivorous and they eat marine algae, worms, clams and other mollusks and sponge. Some foods to add are LRS herbivore diet, small plankton, hikari clam on the half shell and chopped scallopsI'm feeding it only frozen, mostly mysis and it eats a lot. I've only had it for 2 weeks.
