Super skinny I just introduced another tang and the bristle tooth was not eating before I soaked garlic in the food to stimulate its hunger but it did not eat.

garlic does not entice to eat as claimed. Mainly for immunity health.
Unfortunately this fish is moribund and will likely not eat in its' state of health and quite frankly may or not make it.
When did this behavior develop?
What is age of tank?
How long have you had this fish ?

You can add an air stone to increase oxygen, but may not make a difference at this point

my tank is about 4 months old

I itrodused a sailfin tang yesterday and he was not eating before that I purchased him about 3 weeks I was assuming I will make it because he would have died way before this.

I had the same thing happen with my bristle tooth. Was doing fine for awhile eating nori then stopped wanting to eat. Died pretty quickly but right after it died my other fish started showing symptoms of what ended up being ich.
I think it brought it in from the store. I didn't have a quarantine system.....I do now!

Sailfin tang did not even bother it. It might have put stress on the two spot tang because it was bigger.

Fish can go weeks without food and not die.. they are still absorbing water

I would be watching your other fish closely in case there is something communicable.
