Mysterious Fish Deaths

Background 45Gallon red sea Reef Tank that was set up 3 months ago as an upgrade from a pre-existing tank, Frogspawn Heavy with a lot of Zoas
Inhabitants:6 line wrasse, snails and hermit crabs
Question: I keep experiencing fish death whenever I add any new fish in the tank, I have added 3 clownfish which have all perished. The six line wrasse has never shown any signs of aggression and is the only tank inhabitant. I have added 3 clowns and 1 Goby which have all died within 3 days-2 weeks of adding them. Can anyone give me any guesses on what could be going on? Could it be too much flow, chemistry issue, six line issue?

If I had to guess, it's either a disease(unlikely since you have a 6 line alive and kicking), or it's the six line.

My money is on the six line. They are murderous bastiches! You may never see them harass another fish, but they do, and the do to the death.

That is the one fish(along with pseudochromis(dottybacks)), that will never go in any reef I keep again!

Super strange scenario
Ive had 3 clowns Die on me, The six line is never physically seen harassing the fish and just randomly the clowns die and disappear in the tank

Any pics of fish when they were alive?
Also age if tank and how were they acclimated/introduced into the tank?
At the store during purchase, we’re you able to see them eat, breathing and overall swimming behavior?
If not, keep this in BBB mind in the future.
I suspect one of the things below as cause:
- ammonia spike
- low oxygen
- Brooklynella disease

What type of water are you using- RODI or tap water from the faucet ?

temp 77-79
Salinity. 1.025
Ph 8.1 - 8.3
Ammonia .03
Nitrate .04
Phosphate .04

I have been using RODI water we just recently moved and I finally installed a RODI unit from BRS, I called the BRS representative about a question regarding my house water being softened by a water softener before going into the RODI unit and he said thats okay. Could that cause trouble?
