My hammer polyp decided to bail. I have the heads in a plastic container right now.

What should I do for best chances of survival?

I haven't had this happen, but when it has happened to other reefers, the survival rate is nil. It's bailing out to hopefully find a better environment than what it finds itself in. In a closed system like our reef tanks, there really isn't much change from one side of the tank to the other.

I hope you it works out for you, but don't get your hopes up.
