PBT has been scratching, and the grouping is weirdly close together. What are you all thinking? Ich?

This is a skin irritation that is showing mucus cones common with this and hepatus tangs and may be mixed in with ich. Best treatment is in a separate treatment tank using coppersafe at therapeutic level 2.25 for 30 days (do not interrupt this 30 day period) monitored by a reliable Copper Test kit such as Hanna Brand- No API brand. Also monitor Ammonia levels while in quarantine with a reliable test kit and add aeration during treatment using an air stone. other fish often will not show signs but keep an eye on them for any signs and consider treating them also

Ich trophonts settlement is more random than that. These are mucus cones/plugs. They are caused by skin irritation, but that can also have some Ich mixed in. How long have Ho had the tang? Was it fully quarantined with copper?

I just inspected it closer this morning, and it definitely is coned shaped. The pbt has been set up in an established tank for over a year and a half, but I recently moved him tanks. Was afraid the stress maybe triggered an event

Stress, water quality and even injury can be triggers.
