Should I be worried using this ? 15 gal tank . Stand is open in the middle .

I don't know, looks pretty risky.

Does it feel stable with the 15g of water?

I assume you are going to add rocks and/or sand, which will increase the weight a lot more.
 
It's because that has been specifically designed to hold a fish tank like that, where yours has not.

It's really a simple fix, you just got to add 1 or 2 pieces of wood into the center to help support it.
 
One more question. Run the board horizantal all the way across or just 2 smaller vertical ones in the middle ?
 
Center brace from front to back so it doesn't sag in middle

Gently rock it. It should be solid and not swaying or making creaking sounds
 
Would this be sturdy enough ?

It's hard to tell, but it does, if it is real wood.

Try sitting on it, see how it does.

I have my 15g w/ rock and sand on a nightstand similar to that with no problem.
