Almost impossible to ID in that blue light. Try using a flashlight on it, or ramp your whites up for the pic.

Idk, it looks like some foreign object got into the tank. Fabric or stuffing of some sort, possibly filter floss if you're using that?
How many spots do you have that look like that? Have you tried removing it? How quickly does it return?

Are you by chance using NoPox or carbon dosing?
I was just dealing with what looks like the same thing and it was from not adjusting down my NoPox down as I dropped my Phos down. It may still be the same though. If you grab it by hand and it comes out like a wet booger, white in the water, clear to pinkish when its out of water, then its the same and bacterial. Its just an imbalance somewhere in the biome and sometimes you can find the cause and sometimes you just have to let things battle it out as part of an ugly phase.
