Do you have a QT tank setup?

it will be best if you can place this fish is a seperate QT tank where you can apply meds, which are often not reef safe.

What other fish are in the tank and how long have they been together? Have you had parasites in the tank previously?

It does appear to possibly be parasitic and if so will need a QT setup to treat. Is it on both sides of the fish? Are the spots consistent or have then come and gone or moved around on the fish? Another possibility is mucus plugs or lymphocystis. But more info needed to narrow down to something other than ich.

Do you have a QT available and meds on hand?

From the brief video, the fish isn’t breathing too fast so you can rule out velvet. Are the spots on both sides of the fish or just it’s right side?

Hi, based on the video, this isn't one of the major parasites. It is either a bacterial infection, or excess mucus. The fish does seem to have a pinched belly, is it eating well?

Again, I'm only seeing the right side of the fish, are there spots on the left side?

Sorry, with a fish not eating for a month, it will have used its liver for food energy. If you even get it feeding again, the liver damage may be too great for it to survive.

Did the fish develop the spots and then stopped eating, or did it stop eating and then the spots developed?

I do not think that the spots you are seeing are actual parasites, they are mucus from the fish's skin. That can be caused by parasites, but also can be caused by internal disease or general stress.
