I've never seen it anywhere near that large! I'm pretty floored! Glad you have experience here!Could be Hexamita (Hole In The Head Disease). You are probably familiar with this disease, as Discus are also susceptible. It's in the right spot, but I've never seen lesions from Hexamita as big as that before.
Hexamita has another symptom: white stringy poop. So, I'd feel more comfortable with my diagnosis if you were to see that as well.

I reiterate, I'd like to know if the fish has stringy white poop before saying 100% it's Hexamita.
that really looks like it's a growth or something outside of the head/skin. Not an actual hole. I'm looking at the pictures a bit closer and wondering if you observe the same sort of thing here @bif24701 ?
Is he displaying any other symptoms? Like the white stringy poo, or even heavy breathing, flashing or scratching, or hiding alot, or maybe lack of appetite? Anything else that might be a little out of the "norm" ? I'm really looking for additional clues to add to the picture.
It could be an infected wound. Kanaplex + Furan-2 + Metroplex in a QT would be my recommendation for this. Maybe also feed metro soaked food just in case.
