What's this thing growing on my cleaner shrimp?

It's been there over a month, slowly getting bigger. what the heck is it?

isopod, this requires a surgery. sharp blade to cut the sack open and using tweezers pull the isopod out.

My cleaner shrimp has the same thing. I found a video of a guy performing surgery to remove it:

I read about these, when I had a shrimp with one on, I left it.
They tap into the reproduction system in some way so the shrimp can't reproduce properly.
Surgery seems really extreme to me.
