My RBTA did split 2 days back and yesterday I noticed this one with strange looking mouth.
Whats wrong with it?

I did water change yesterday.

If it just split give it a bit of time. Looks like it's still healing. Sometimes their mouth can do that when they're healing from a split.
 
Depends. Anemone will split for the 2 extremes. They will split if they're getting everything they need and want to reproduce. They will also split as a latch ditch effort at survival. Which category you fall into will dictate what's going on here.
