Wrasse with back damage

I have had this wrasse over 2 years. 6 months ago i had ich and quarantined. Then they were back in this tank with no issues. I came back after a week and my naokos wrasse is completely bent and is struggling to use its tail. As of now i have taken of my powerhead and halved the main flow. Should i move it to a quarrntine tank and can it heal or is its fate basically sealed at this point. As i do know this could also be unknown neurological wrasse disease. He has some nose and tail damage that would also indicate fighting for some reason.

I think that might be UNWD... I'm sorry. But you should ask @Jay Hemdal

The bumped snout is weird though. You are right about that.

Is it still eating/going trying to go for food?
 
I haven’t tried food as it is quite late over here but it can swim perfectly well if it wants to get somewhere
 
Also i thought that was only in new wrasses, or can they get it at any point?
I think they can get it at any point... But the fact that he can swim just fine is weird. Hopefully Jay can provide some insight.

I’ve only seen UNWD in recently imported wrasses. Not sure what’s going on here.
 
Well whatever it is, it has taken its toll and i cant find the fish this morning when i was planning to transfer it over to a hospital tank. Thanks all for your help
 
I lost a pintail wrasse when it hit the glass cross brace on my tank it couldn’t swim and died the next day. My wrasses like to chase each other and usually they hit the mess netting with no issues but this time it smacked the brace.

Yeah thats probably what happened here but maybe swimming into the glass instead as my lid has no brace just an aluminium frame
 
What are the chances it broke its back from trauma (jumping or hitting a rock too hard)? Or would a small fish not really have enough force for that?

With true UNWD is often misdiagnosed as a strike injury. However, there is never any damage seen to the fish’s face, and you’re correct, the mass of these fish is so small, that sort of jury seems less likely.
I’m not sure in this case though…..
 
Update again, i found him after feeding the tank and moved it over to the hospital tank. It has very little control now, seems like it is trying to eat but cant manage. Could it be a swim bladder issue? It does move around the tank sometimes but is often upside down
 
Is there anything i can do at this point or should i just do the right thing and put it out of its misery?
